Your Role in Our Mission

As a vital member of Cleveland Clinic’s Social Work team, you will provide compassionate, patient-centered social work services. Your work will help meet essential human needs, empower clients to achieve their optimal psychosocial functioning, and enhance overall well-being. This position is crucial for delivering quality psychosocial care to patients receiving cancer treatment at our regional sites.

What You’ll Do

In this day shift role (8:00 AM to 5:00 PM), you will:

• Collaborate with healthcare teams to assess and plan interventions that help patients navigate social, emotional, financial, and environmental challenges.
• Conduct psychosocial assessments and provide interventions, including counseling, education, advocacy, conflict resolution, and crisis intervention.
• Document treatment recommendations and care coordination interventions to ensure comprehensive patient care.
• Engage patients and families in adhering to treatment plans by identifying and reducing barriers.
• Maintain a robust network of community resources to connect patients with necessary support.
• Supervise social work interns and assist in training new caregivers.
• Educate fellow caregivers on the impact of social issues on health outcomes and the effectiveness of social interventions.

What We’re Looking For

To thrive in this role, you should have:

• A Master’s degree in social work from a CSWE accredited program.
• A State Licensed Social Worker (LSW) certification.
• At least two years of experience in a hospital or healthcare setting.
• A commitment to obtaining a Licensed Independent Social Worker (LISW) certification within five years of hire.
